How to refer

Sutton Primary Liaison and Recovery Service operates as a single point of access for all referrals for services offered by Sutton Uplift.

If you have any queries about your referral, please get in touch using the details in the Contact Us section of the website.

Who Can Be Referred?

Our service is available for people who are aged 18 and over who live in Sutton or have a Sutton GP.

Self referrals

Please use the Self Referral Form.

GP Referrals

Referrals from all professionals working in GP practices in Sutton must be submitted via e-RS and will not be accepted if sent via email or using the forms on this webpage

Out of area of area GP referrals should be made using the Professional Referral Form

Professional referrals (external organisations)

Referrals from external professionals (for example, hospitals, third sector organisations, out of area GP practice staff etc.) should be made using the Professional Referral Form

Professional referrals (internal - South West London and St Georges teams)

All internal referrals should be made following internal referral processes. Any internal referrals that are made using the Professional Referral Form on this webpage will not be accepted.
